This index was originally compiled in 2001 by the Central Massachusetts Business Environmental Network (CMBEN) and the Office of Technical Assistance, with assistance from MassDEP.  It was updated by MassDEP in February 2008 and identifies the key environmental regulatory thresholds that businesses need to be concerned about - across all of DEP's programs.  The Index is intended for informational purposes only and is not a substitute for reading and complying with the full text of state and federal regulations. Please contact the respective local, state, and federal agencies for further information.

This form should be used when submitting supplemental information or payment to a previously filed permit and must include the transmittal number of the original submission.
